Three things together: a toe box shaped like a foot rather than a wedge, a sole that sits flat from heel to forefoot (zero drop), and an outsole thin and flexible enough to feel and bend with the ground. Take away any one of them and you have a light sneaker, not a barefoot shoe.
Your feet should feel fine; your calves may not. Removing the heel ramp hands work back to the Achilles and calf that a cushioned shoe was doing for you. Build up over two to four weeks and the ache passes. Jumping straight into a 10K is how people conclude, wrongly, that barefoot shoes do not suit them.
Deliberately, no. The arch is a spring held up by muscle, and support that never lets it load keeps it weak. Most styles ship with a thin removable insole so you can soften the first weeks and then take it out.

That is more or less the design brief. The forefoot is generous on every last, so wide feet usually stay at their normal length. High insteps are the case to watch — lace-ups adjust, slip-ons do not.
Canvas and mesh uppers generally survive a cold, gentle cycle in a laundry bag with the insoles taken out. Air-dry them away from radiators: heat is what shrinks glue lines and curls thin soles. Leather-look uppers prefer a damp cloth.
Less than a slab of foam, honestly — there is less material there to wear away. Rotating two pairs and keeping them off hot asphalt in summer buys a surprising amount of extra life.
Yes: drainage ports, quick-dry linings and grippier rubber for wet rock. They are built to be soaked and rinsed, and they dry fastest with the insole pulled out.
